This is changing rapidly. This chapter draws on recent evidence to provide an overview of best practice in assessing and identifying supportive care needs and in planning ahead to ensure optimal care for the future (advance care planning). Without advance care planning, deteriorating health is often poorly managed for those with advanced CKD, and care is not in accordance with the preferences of the individual person affected.
