Abstract
This chapter will consider a range of health and social care assessments upon which care and support of adults with learning disabilities may be based. By continuing our journey with Jeremy, we’ll consider how he came to the situation described above, what happened when he became acutely unwell and how he was supported to go forward to the next phase of his life.
